More than a half-century ago, the American Academy of Pediatrics recognized the value of creating a “medical home.”1 This concept emphasizes the coordination of care across various disciplines. At first, the focus was on coordinating care for people with “special health care needs.” Over time, this concept has evolved into a broader vision of a patient-centered health home.2
