I am writing regarding the June guest editorial entitled “Value-Based Care in Dentistry: Is the Future Here?” (Vujicic M, David G. JADA. 2023;154[6]:449-452). As one of the dinosaurs of our profession (43 years in practice and multiple states and American Dental Association positions), value-based care, sadly, sounds like the many capitation plans that failed miserably in the 1980s and 1990s. If the goal is to reach category 4 in the guest editorial’s figure, the only motivation for practices is to not treat the patient, or treat with the least expensive alternative, even when that may not be in the patient’s best interest.
